CFW’s staff is happy to manage the insurance claim processing on your behalf. We accept payment from most insurance providers, and we are in network with Horizon (and other state/federal) Blue Cross Blue Shield, Beacon Health, Emblem/GHI, Optum/United/Oxford, and Cigna at present.

If your insurance is not listed, please call us. Co-payments are payable when services are rendered.
