To ensure transparency and ease your concerns, our staff checks your insurance benefits for you. During your initial phone consultation, our staff will ask for your insurance information. Prior to your initial session, we will verify your benefits and determine any potential deductible or other out-of-pocket requirements based on your coverage. During your assessment, or sooner, we will advise you of what your insurance has told us they will cover and any amount(s) for which you will be responsible.

CFW’s staff is happy to manage the insurance claim processing on your behalf. We accept payment from most insurance providers, and we are in network with Horizon (and other state/federal) Blue Cross Blue Shield, Aetna, Beacon Health, Emblem/GHI, Optum/United/Oxford, and Cigna at present.
WE ACCEPT PAYMENT FROM ALL COMMERCIAL INSURANCE AND ARE IN NETWORK WITH THE MAJORITY OF THE LIST SHOWN.

If your insurance is not listed, please call us. Co-payments are payable when services are rendered. If you have questions on insurance, billing, co-payments or any other financial matters, please contact us so we can assist you promptly.
